  • Purpose of use and maximum expected number of people
  • Name of the user [Company name] / [Zip code] / [Address] / [Telephone number
  • Name on signboard in front of the meeting room
  • Name of Receipt Recipient
  • Other (e.g., request for free rental of projector or screen, etc.)
